Zusammenfassung
Während bis vor wenigen Jahren die Pharmakokinetik für die medikamentöse Therapie für klinisch wenig relevant gehalten wurde, ist nunmehr ihre Bedeutung für einen sachgerechten Einsatz von Medikamenten unumstritten. Die Wirksamkeit eines Pharmakons hängt nämlich nicht nur von seinen pharmakodynamischen, sondern auch in hohem Maße von seinen kinetischen Eigenschaften ab. In Abb. 1 sind die nach Applikation eines Parmakons im Organismus ablaufenden Vorgänge schematisch dargestellt. Es leuchtet ein, daß Organerkrankungen mit Einfluß auf das pharmakokinetische Verhalten die Wirksamkeit eines Pharmakons wesentlich beeinflussen können. Aufgrund der zentralen Rolle bei der Ausscheidung von Arzneistoffen wirken sich Veränderungen der Nierenfunktion besonders deutlich aus. Dies gilt naturgemäß auch für Antihypertensiva. Während bislang kein Einfluß einer Nierenkrankheit (außer der Urämie) auf die Resorption von Pharmaka nachgewiesen wurde, sind das Verteilungsvolumen und die Plasmaeiweißbindung von der Art und dem Ausmaß der Nierenerkrankung abhängig. So können z. B. renale Ödeme Extrakompartimente bilden. Die Plasmaeiweißbindung eines Pharmakons beeinflußt sowohl dessen Pharmakodynamik — nur die nicht-proteingebundene Form ist wirksam — als auch die Eliminationskinetik. Nehmen z. B. beim nephrotischen Syndrom die Plasmaalbumine ab, so kann auch die Eiweißbindung abfallen und damit — zumindest vorübergehend bis zur Einstellung eines neuen Steady state — die freie, das heißt wirksame Form des Pharmakons zunehmen. Bei der renalen Elimination ist nur die glomeruläre Filtration von der Proteinbindung abhängig, nicht dagegen die tubuläre Sekretion. Sinkt der eiweißgebundene Anteil im Plasma, so kann mehr Wirkstoff glomerulär filtriert werden.
