Zusammenfassung

Die Anwendung kontinuierlicher oder zyklisch variierender positiver Atemwegsdrucke, d.h. über den umgebenden Atmosphärendruck hinausgehender intratrachealer und damit auch intraalveolärer Drucke (Palv), ist ein akzeptiertes Verfahren bei der Behandlung von Störungen des pulmonalen Gasaustausches und/oder einer Insuffizienz der Atemmuskulatur. Das technische Spektrum reicht dabei von Beatmungsfrequenzen zwischen 6 (intermittierende Überdruckbeatmung, IPPV) und 4000 min−1 (High frequency oscillation) mit einer entsprechenden Anpassung des Tidalvolumens (VT) zur Erzielung einer Normokarbie, über die intermittierende mandatorische Beatmung (IMV) und Spontanatmung mit Druckunterstützung (ASB), bis hin zur reinen Spontanatmung. Dabei kann den dynamischen intratrachealen Druckschwankungen durch das Beatmungsgerät jeweils zusätzlich ein statischer kontinuierlicher positiver Atemwegsdruck (CPAP) überlagert werden. Der nachstehende Beitrag bezieht sich zwar insbesondere auf die Wirkungen von kontrollierter maschineller Beatmung (CMV) mit positiv endexspiratorischem Druck (PEEP) bzw. CPAP auf die Lungen- und Herz-Kreislauf-Funktion, jedoch gelten die meisten Überlegungen prinzipiell auch für andere Formen der Atemtherapie und Beatmung.
