Zusammenfassung

Thrombotische Komplikationen stellen in der operativen Medizin ein erhebliches Problem dar. Sie treten häufig auf und können eine lebensbedrohliche Lungenembolie verursachen. Bei mehr als 30% aller operierten Erwachsenen tritt ohne Thromboembolieprophylaxe eine tiefe Beinvenenthrombose auf, bei etwa 5% der Patienten kommt es zu einer Lungenembolie, und bei 0,1–1% verläuft diese Lungenembolie tödlich. Auch das postthrombotische Syndrom, das in der Regel erst einige Jahre nach der Thrombose auftritt, hat durch seinen rezidivierenden Verlauf erheblichen Krankheitswert. Thrombosen und Embolien sollte deshalb in der perioperativen Situation konsequent vorgebeugt, wenn sie auftreten, sollten sie konsequent behandelt werden. Die prä- und intraoperative Thromboseprophylaxe und die Notfall- und Intensivtherapie der Lungenembolie fallen in den Arbeitsbereich des Anästhesisten.
