Zusammenfassung
Das der Ulkuskrankheit zugrundeliegende peptische Geschwür ist Folge des Ulkus: Ungleichgewicht Überwiegens von aggressiven über schleimhautschützende Faktoren. Neben zwischen Säure und einer, vornehmlich beim Ulcus duodeni, vermehrten Säure- und Pepsinsekretion Schutzfaktoren kommt der Verminderung mukosaler Schutzfaktoren und der Störung ihrer Funktion eine wesentliche Bedeutung bei der Ulkusentstehung zu. Zu den Schutzfaktoren zählen die optimale Schleimzusammensetzung, Zellregenerationen, die mukosale Durchblutung, die Prostaglandine und das Urogastron. Zur Entstehung eines Ulcus ventriculi tragen wesentlich die Irritation des Epithels durch exogene Noxen, Gastritis, Gallereflux, antrale Stase und Defekte der Schleimzusammensetzung bei. Gastritis und Ulkuserkrankung können auch mit einer Besiedlung von gramnegativen Helicobacter (Campylobacter) pylorii einhergehen. Der Arzt sollte vor jeder medikamentösen Behandlung die Diagnose „Ulkus“ einwandfrei belegen. Ulzera heilen in der Regel spontan aus. Dieses erschwert die Beurteilung des therapeutischen Erfolges. Die Diagnose „Ulkus“ bedeutet einen Defekt der Schleimhaut mit Beteiligung der Lamina muscularis mucosae bzw. tieferer Abschnitte. Bei Erosionen ist nur die obere Epithelschicht betroffen.
