Morbus Crohn Colitis ulcerosa

Zusammenfassung

Zahlreiche Erkrankungen des Dünn- und Dickdarms sind hinsichtlich ihres klinischen Verlaufes, der Laborparameter und des endoskopischen und histologischen Erscheinungsbildes der Colitis ulcerosa und dem Morbus Crohn ähnlich. Dies bedeutet, daß die Reaktionsmöglichkeiten des Darmes auf unterschiedliche Schädigungen begrenzt sind. Die Diagnose einer chronisch-entzündlichen Darmerkrankung ist deshalb nur auf dem Boden einer genauen Anamnese und klinischen Untersuchung, einer vollständigen Endoskopie und einer sorgfältigen histologischen Aufarbeitung der Biopsien unter Berücksichtigung der zahlreichen Differentialdiagnosen möglich. Es gibt weder für den Morbus Crohn noch für die Colitis ulcerosa eindeutig beweisende klinische, endoskopische oder histologische Kriterien. Von daher bleibt die Diagnose eine Ausschlußdiagnose, die um so sicherer ist, je zuverlässiger die Differentialdiagnostik durchgeführt wird (Tabelle 17).
