Middle Molecules in the Serum of Patients and Rats with Liver Failure: Influence of Sorbent Haemoperfusion

Abstract

Increased concentrations of middle molecular fractions (m.w. 1000–1500 daltons) could be detected in ultrafiltrates of serum samples from patients with acute liver failure (fulminant viral and Amanita phalloides intoxication) and with liver cirrhosis after Sephadex G 15 gel filtration. Similar fractions could be demonstrated in the serum of galactosamine pretreated rats (2 g/kg b.w.). At least some of the fractions contain peptides. Sorbent haemoperfusion significantly reduced the middle molecule concentration of the serum. It is concluded that different forms of liver failure have in common the accumulation of middle molecular solutes. However, their biochemical and clinical relevance have to be evaluated using more specific determination methods, including toxicity tests.
