Zusammenfassung
Der Effekt der äußeren Atmung wird von vier Teilprozessen bestimmt, die sich in ihrer Wirksamkeit gegenseitig beeinflussen können. Auf den kürzesten Nenner gebracht, lassen sie sich als Ventilation, Perfusion, Diffusion und Distribution kennzeichnen (s. Abb. 1). Die alveoläre Ventilation bestimmt die Sauerstoffmenge, die pro Zeiteinheit in die Alveolen transportiert wird, und die Kohlendioxydrate, die von hier aus an die Umgebungsluft abgegeben wird. Damit stellt die Ventilation einen wesentlichen Faktor für die Einstellung der austauschbestimmenden O2- bzw. CO2-Drucke in den Alveolen dar. Die Perfusion der Lunge ist maßgebend für den Antransport des Kohlendioxyds und den Abtransport des Sauerstoffes. Sie bestimmt also ganz wesentlich die O2- bzw. CO2-Drucke in den Lungencapillaren. Die Diffusionsgröße schließlich entscheidet über die Gasmengen, die bei gegebenen Druckgradienten zwischen Alveole und Lungen-capillare ausgetauscht werden. Für ein kleines Austauschgebiet ist der Arterialisierungseffekt durch diese drei Größen vollständig festgelegt. In bezug auf die Leistung der gesamten Lunge hat man jedoch noch einen vierten Faktor zu berücksichtigen. Wir wissen nämlich, daß schon beim Gesunden, in verstärktem Maße aber unter pathologischen Bedingungen, Ventilation, Perfusion und Diffusion nicht gleichmäßig über alle Lungenabschnitte verteilt sind. Diese ungleichmäßige Verteilung oder Distribution kann den Arterialisierungseffekt entscheidend mit beeinflussen und darf daher bei einer Beurteilung der Lungenfunktion nicht vernachlässigt werden.
