Advanced Preclinical Emergency Care — Helicopter Medical Team

Summary

Decreased morbidity and mortality due to preclinical emergency care has been well documented. We present a review of 1546 consecutive patients treated and transported by our helicopter emergency medical team between 1986 and 1989. A total of 1085 (70.2%) were trauma patients and 54.3% of these were treated ot the scene of the accident. Some 47.4% of the patients scored 5, 6, or 7 on the NAC A index, i.e. were severely injured. The low mortality before and during the flight (3.0% and 0.3% respectively) confirms our treatment concept of applying ICU principles at the scene of the accident.

Zusammenfassung

Die Vorteile einer Notfallmedizin mit Beginn der Therapie am Unfallort sind längst belegt und führen zur Senkung von Morbidität und Letalität. Wir präsentieren eine Analyse von 1546 konsekutiven mittels Rettungshelikoptereinsatz zwischen 1986 und 1989 versorgten Patienten. 1085 (70,2%) waren Traumapatienten, wovon 54,3% im Rahmen eines Primäreinsatzes versorgt wurden. 47,4% der Patienten wiesen einene NACA-Schwergradindex von 5,6 oder 7 auf. Die Letalitätsrate vor resp. während des Fluges (3,0 resp 0,3%) war ausgesprochen tief und spricht für die Richtigkeit unseres interventionellen Konzeptes mit Therapieeinleitung am Unfallort zur Kreislauf- und Beatmungsstabilisation und zu adäquatem Monitoring.
