Summary
The incidence of chronic inguinal pais as a comphcation of hernia repair was shown as part of a prospective study. Since 12/87,1099 Shouldice repairs, 770 primary hernias (PH), and 329 recurrent hernia (RH) were studied. Therefore, at least 1 year after operation (median 16.2 month) the patients underwent a standardized interview and a physical examination with a follow-up rate of 89.7% for PH and 88.7% for RH. In the control (c) group, (CPH, n = 231; CRH, n = 2) postoperative inguinal pain was found in 4.2% of PH and 6.4% RH. A precise preparation of the genital branch of genitofemoral nerve, performed in 554 of the PH and 235 of the RH reduced the rate of inguinal pain (PH, 1.4%; RH 2.6%) significantly (p < 0.05), without elevating the rate of other comphcations or of recurrences. Complications CPH 5.5% to PH 7.0%, and CRH 11.7% to RH 11.2%. The recurrence rate was CPH 0.5% to PH 1.4% and CRH 3.2% to RH 3.0%. The syndrome of R. genitalis is as important as the syndrome of N. ilioinguinalis and can be avoided by precise preparation, routine separation, and facultative severance of the genital branch of the genitofemoral nerve in the deep inguinal ring.
Zusammenfassung
Im Rahmen unserer prospektiven Leistenhernienstudie (12. 85 bis 2. 91 1471 Shouldice-Reparationen) wurde der chronische Leistenschmerz nach 1099 Shouldice-Reparationen (770 Primärhernien [PLH] u. 329 Rezidivhernien [RLH] untersucht. Methodik: Shouldice-Reparation, jahrgangsweise Nachuntersuchungen mit standardisierter neurologischer Diagnostik (Median 16,2 Monate p.op.). Follow up PLH 89,7% und RLH 88,9%. Chronischer Leistenschmerz fand sich in der Kontrollgruppe (K-PLH n = 237; K-RLH n = 112) bei PLH mit 4,2% und bei RLH mit 6,4%. Gezielte schonende Darstellung am inneren Leistenring (Präparation, Ablösung und fakultative Durchtrennung des R. genitalis des N. genitofemoralis) erfolgte bei PLH n = 554 und RLH n = 235 mit signifikant reduzierter Inzidenz des chronischen Leistenschmerzes (PLH 1,4%, p < 0,05 u. RLH 2,6%, p < 0,05). Komplikationsquote (K- PLH 5,5% zu 7,0%; K-RLH 11,7% zu 11,2%) und Rezidivrate (K-PLH 0,5% zu 1,4%; K-RLH 3,2% zu 3,0%) bleiben durch die modifizierte Präparationstechnik unbeeinflußt. Für die Ursache des chronischen Leistenschmerzes ist das Ramus Genitahs-Syndrom ebenso wichtig wie das Ilioinguinalis-Syndrom. Die schonende Präparation des R. Genitalis am inneren Leistenring ist dringlich zu empfehlen.
