Value of Indwelling Shunts in Carotid Surgery — Hemodynamic Reserach by Transcranial Doppler Sonography(TCD)

Summary

Transcranial Doppler sonography was used in 43 patients undergoing carotid endarterectomy (CEA). Mean flow velocity in the middle cerebral artery (MV MCA) was measured before, during and after surgery. MCA MV was significantly reduced by clamping for shunt installation (39.9 to 17.5 cm/s) and for shunt removal (42.5 to cm/s). While the shunt was in operation MCA MV was regularly as high as it was pre or postoperatively. In cases of contralateral stenosis of more than 70% the decrease of MCA MV was significantly more pronounced (—31.2 cm/s for installation and 32.9 cm/sforremovalthan without a contralateral stenosis (—15.6 cm/s or 18.1 cm/s). Especially in multivascular desase shunting can reduce ischemic comphcations

Zusammenfassung

Bei 43 Patienten wurde die Blutflußgeschwindigkeit in der A. cerebri media vor, während und nach Carotis-TEA mit Shunteinlage gemessen. Es fand sich ein signifikanter Flußabfall in den Abklemmphasen zur Einlage (von 39,9 auf cm/sec) und Entfernung (von 42,5 auf 17,4 cm/sec) des Shuntes. Bei laufendem Shunt wurde das prä- bzw. postoperative Niveau erreicht. Bei kontralateraler Stenose über 70% war der Flußgeschwindigkeitsabfall mit 31,2 cm/sec (Einlage) und 32,9cm/sec (Entfernung) signifikant ausgeprägter als ohne kontralaterale Stenose (15,6 bzw. 18,1 cm/sec). Insbesondere bei Mehrgefäßerkrankung kann der Shunt zur Verminderung ischämischer Komphkationen beitragen.
