Divertikulitis bei Niereninsuffizienz: Prophylaktische Operation vor Nierentransplation?

Summary

Between January 1975 and February 1991, a totol of 1952 renal transplantations (RTs) were performed in 1639 patients (pts.). Because of a history of diverticulitis (D) 18 pts. underwent prophylactic surgery prior to RT without severe comphcations. Post-RT, the incidence of colonic perforation (CP) caused by D was 0.7% (11 pts.) overall and 2% in pts. >55 years. The risk tended to decrease slightly under a reduced immunosuppressive regimen instituted in 1987. Five CPs occurred within 2 weeks post-RT, another three within 3 months. Despite the finding of severe peritonitis during surgery, there was a lack of clinical symptoms, resulting in a mean delay of diagnosis of 2 days. Four of these pts. died (36%). Pre-RT, prophylactic surgery is recommended in pts. with a history of diverticulitis.

Zusammenfassung

Von 1/75 bis 2/91 wurden 1952 Nierentransplantationen (Tx) bei 1639 Pat. durchgeführt. Vor Tx wurden 18 Pat. wegen bekannter Divertikuhtis prophylaktisch operiert, ohne Letalität oder schwere Komphkationen. Nach Tx erlitten 11 Pat. (0,7%) eine Divertikelperforation. Die Häufigkeit war mit dem Alter ansteigend (> 55 J: 2%). Unter reduzierter Immunsuppression (seit 1987) war das Risiko etwas geringer. 5 Perforationen ereigneten sich in den ersten 2 Wochen, weitere 3 bis zum 3. Monat nach Tx. Trotz schwerer Peritonitis bestand meist nur eine geringe Symptomatik, im Mittel war die Diagnose deshalb um 2 Tage verzögert. 4 Pat. verstarben (36%). Für Pat. mit Divertikulitisanamnese wird eine prophylakt. Op empfohlen.
