Summary

From January 1981 to March 1991, 191 patients with ulcerative colitis (UC) were surgically treated in the Department of Surgery of the University of Heidelberg [failure of conservative treatment n = 119, emergency n = 24, carcinoma n = 14 (7.3%), others n = 34]. More than 50% of patients underwent a continence-preserving procedure [ileal pouch anal anastomosis (IPAA) n = 97, ileorectal anastomosis (IRA) n= 18]. In 26 patients proctocolectomy with terminal ileostomy was performed. Local septic complications and intestinal obstruction were comparable for all procedures.Ten patients died (toxic colon n = 4/18, carcinoma n = 4/14, primary sclerosing cholangitis PSC n = 2/8). The procedure of choice is the IPAA. Early surgical treatment in UC means prophylaxis in different respects: reduction of complications and long-term steroid treatment, early detection and therapy of cancer, improvement of extraintestinal manifestations, and improvement of quality of life.

Zusammenfassung

Von 1/81-3/91 wurden 191 Patienten wegen CU behandelt (therapierefraktäre Situation n = 119, Notfall n = 24, Carcinom n = 14 (7,3%), andere n = 34). Über 50% wurden kontinenzerhaltend operiert: Ileoanale Pouchoperation (IAP) n = 97, ileorektale Anastomose (IRA) n = 18; Proktocolektomie mit terminalem Ileostoma n = 26. Lokalentzündliche Komplikationen und postop. Ileus waren jeweils gleich häufig. Es verstarben 10 Patienten (toxisches Colon n = 4/19, Carcinom n = 4/14, Primär sklerosierende Cholangitis PSC n = 2/8). Therapie der Wahl ist die IAP. Die rechtzeitige chirurgische Therapie der CU ist Prophylaxe im weiteren Sinne bezüglich chirurgischer Komplikationen, Medikamentenfolgen, Carcinomfrüherkennung und Therapie, Beeinflussung extraintestinaler Manifestationen, Verbesserung der Lebensqualität.
