Zusammenfassung

Die Anwendung ionisierender Strahlen in der Therapie der malignen Mastdarmgeschwülste ist nicht unbestritten und nach Auffassung vieler Chirurgen nutzlos. (Im neueren deutschen Schrifttum s. hierzu Hellner, 1953; Guleke, 1957; Kootz, 1959.) Dem steht die Erfahrung vieler Strahlentherapeuten entgegen, daß man bei der so großen Gruppe der nicht radikal operablen Kranken bemerkenswerte Palliativerfolge erzielen und zuweilen sogar Operabilität erreichen kann. Wenn es auch außer Frage ist, daß die Operation bei einem ausgereiften Krebs des Darmes an der Spitze aller therapeutischen Bemühungen steht und auch weiterhin in absehbarer Zeit stehen wird, so hat doch die Strahlenbehandlung in einer jahrzehntelangen Entwicklung ein Stadium erreicht, das es als richtig erscheinen läßt, sie mehr als bisher in die therapeutische Diskussion miteinzubeziehen. Die Probleme der Kombinationsbehandlung und der präoperativen Strahlentherapie wurden erstmals unter einheitlicher Auswertung eines großen chirurgischen und radiologischen Krankengutes von Hultborn (1952) behandelt. Dieser Monographie kommt somit auch für die Strahlentherapeuten eine besondere Bedeutung zu. In den nun folgenden kurzen Ausführungen zur Klinik des Rectumcarcinoms stützt sich unsere Darstellung neben den schon erwähnten Monographien von Guleke und Hultborn auf die Bücher von d’Allaines (1956) und Reifferscheid (1962).
