Summary
The metabolism of eostrogens in liver disease is presented in a review and attempts are made to explain the very controversial results hitherto obtained. Some of the results can be explained solely on the basis of poor methodology, since the measurement of oestrogens in liver disease involves many hazards. In addition the results are much depending on the type and the stage of the disease. The changes in oestrogen metabolism detected seem usually not to be due to disturbed metabolism of oestrogens in the liver cells but mainly to changes in the enterohepatic circulation, uptake and transport of the oestrogens. Following oestrogen load different excretion patterns are obtained in subjects with various types of cholestasis, in subjects with chronic liver disease without hypoproteinemia and ascites and in such subjects in which extracellular fluid retention occurs. However, since most investigators have not tried to analyze their clinical material in detail, the results have been confusing. It seems now, that it will be possible to find typical patterns of oestrogen metabolism in various types and stages of liver disease.
