Summary
We refer to the frequent occurence of subclinical hyperplasias or microadenomas in the distal lobe of the pituitary whose size does vary very much. 10% of the intracranial tumours belong to the volume-taking adenomas. Their various extension-possibilities are described. It is noted that the individual strongly marked variation of the anatomical details in the hypophysis region influence the symptomatology of the adenomas.
The histological diagnosis based on hitherto existing staining methods does not give justice to the clinical reality. It is possible by aid of cytochemical methods to classify the adenomas in accordance with clinical statements; either into endocrine-active or endocrine-inactive groups, of which the so called eosinophilic and basophilic adenomas show extreme variances of the active group.
The idea known up to present times that practically no volume-taking basophilic adenomas exist is contradictory. Adenomas proved by histochemical methods give evidence that their cells contain substrate-complexes at changeable frequency which are responsible for the secretion of gonado-thyreotropic hormones.
We come to the conclusion that probably the tumourgrowth caused by interruption of the hypothalamic influence does transform the histological picture and, consequently also the functional behaviour of the adenomas in the sense of an inactivity for which also the distribution of age of both adenoma-groups is responsible, to wit the active group at the age peak of 35–40 and the inactive group at the age between 55–60 years.
