Zusammenfassung

Als Pharynx bezeichnet man den der Halswirbelsäule unmittelbar aufliegenden, von der Schädelbasis bis zum Speiseröhrenmund reichenden Schleimhaut-Muskelschlauch. Seine Länge beträgt beim Erwachsenen durchschnittlich 14 cm. Die Pharynxwand setzt sich aus Tunica mucosa, Tunica submucosa, Tunica muscularis und Fascia pharyngea zusammen. Während im Epipharynx die Tunica mucosa von einem Zylinderepithel mit Flimmerhaaren überzogen ist. findet sich im Meso- und Hypopharynx ein geschichtetes Plattenepithel, welches zahlreiche seröse und muköse Drüsen enthält. Die Tunica submucosa weist stellenweise reichlich lymphatisches Gewebe auf. Eine Anhäufung desselben hegt im Bereiche des sog. lymphatischen Rachenringes (Waldeyer). Die Tunica muscularis setzt sich aus den 3 Pharynxconstrictoren zusammen: M. constrictor pharyngis sup. (M. cephalopharyn- gicus), M. constrictor pharyngis med. (M. hypopharyngicus) und M. constrictor pharyngis inf. (M. laryngopharyngicus). Hinzu kommen die Rachenheber: M. stylo- und palatopharyngicus. Die arterielle Blutversorgung des Rachens erfolgt durch die A. pharyngea ascendens, A. maxü- laris und A. facialis. Alle Venen münden in die V. jugularis interna. Die reichlich vorhandenen Lymphgefäße führen, zum Teil auf Umwegen, zu den Lymphonodi cervicales profundi. Unter den Zwischenstationen spielen die im Kleinkindesalter vorkommenden retropharyngealen Lymphknoten eine besondere Rolle. Die Innervation des Rachens ist durch denPlexus pharyngicus gegeben, an dessen Aufbau der N. vagus, der N. glossopharyngicus und der Sympathicus beteiligt sind. Ein Teil der motorischen Fasern stammt vom Ramus medialis des N. accessorius. Daß sich an der Racheninnervation mehrere Nerven beteiligen, zeigt sich besonders bei umschriebenen Erkrankungen im Bereiche der Hirnnervenkerne, da bei solchen Fällen oft nur geringgradige Ausfallserscheinungen feststellbar sind. Die Durchtrennung des N. glossopharyngicus, z. B. bei Neuralgien dieses Nerven, hat keine bedeutenden Störungen im Pharynx zur Folge.
