Zusammenfassung
β-Blocker reduzieren das Risiko koronarer Ereignisse und die Mortalität von Patienten mit Hypertonie oder abgelaufenem Myokardinfarkt. Die Beweislage für einen lang andauernden Vorteil der β-Blocker in der Therapie dieser beiden Patientenkollektive ist wesentlich überzeugender als für irgendeine andere, bei der Behandlung dieser Patienten eingesetzten Substanzgruppe. Trotz dieser Tatsache werden β-Blocker zu wenig in der Therapie eingesetzt, da Ärzte die β-Blocker bei vielen Risikopatienten als kontraindiziert ansehen. Die Daten zahlreicher Studien zeigen aber, dass vor allem ältere Patienten, Patienten mit Herzinsuffizienz NYHA-Stadium II und III, Diabetiker und Patienten mit chronischen Lungenerkrankungen nicht nur die β-Blockertherapie tolerieren, sondern vielmehr im Vergleich zu weniger schwer erkrankten Patienten einen deutlichen Vorteil von der Therapie haben.
β-Blocker wurden ausgiebig über Jahre sowohl in der Therapie der Hypertonie und der Angina pectoris als auch zur Verbesserung der Prognose bei Patienten mit abgelaufenem Myokardinfarkt eingesetzt. Kürzlich abgeschlossene Studien zeigten ihre Effektivität in der Behandlung von Patienten mit Herzinsuffizienz; es kann vorausgesagt werden, dass β-Blocker in der Zukunft zunehmend in der Behandlung der Herzinsuffizienz eingesetzt werden. Das erste Ziel dieses Beitrags ist es daher, den Wirkmechanismus der β-Blocker in der Therapie der Herzinsuffizienz knapp zu erklären und die Beweise für die Effektivität der β-Blocker bei der Reduktion der Mortalität an koronarer Herzkrankheit (KHK) bei Patienten mit Hypertonie oder abgelaufenem Myokardinfarkt (MI) kurz darzustellen. Das zweite Ziel ist es, die wahrgenommenen Probleme im Zusammenhang mit der β-Blocker-therapie zu erläutern. In den frühen Jahren nach Einführung der β-Blocker, von 1965 bis 1975, beschrieben die pharmakologischen und medizinischen Lehrbücher ausgiebig die wahrgenommenen Risiken einer β-Blockertherapie. Als eine Konsequenz daraus wurden über die letzten drei Jahrzehnte β-Blocker nicht in der Therapie von älteren Patienten, Diabetikern oder Patienten mit Herzinsuffizienz eingesetzt. Kürzlich abgeschlossene Studien haben aber Veranlassung gegeben, gerade diese Bedenken gegen den Einsatz der β-Blocker in der Therapie dieser speziellen Patientenkollektive erneut aufzugreifen. Wir glauben jetzt, dass gerade diese Patientenkollektive, denen bisher eine β-Blockertherapie vorenthalten wurde, wahrscheinlich am meisten von derselben profitieren.
