Zusammenfassung
Verschiedenste Statistiken besagen, daß die hintere Kapselruptur besonders bei erfahrenen Chirurgen in bis zu 80% bei der Kortexaspiration nach der Phakoemulsifikation passiert. Implantiert man aber eine faltbare IOL direkt nach der Phakoemulsifikation in ein Gemisch aus Viskoelastikum und Kortex, wobei man die benötigte viskoelastische Substanz noch zur Dissektion der verbliebenen Linsenrinde verwenden kann, so ergeben sich hieraus mehrere positive Ergebnisse: 1. Kortex- und Viskoaspiration wird zu einem Vorgang zusammengezogen; somit wird aus bisher zwei nur eine Manipulation, die zudem die intraokulare Verweildauer herabsetzt. 2. Große schlaffe, wie auch alle anderen Kapselsäcke werden durch die IOL-Haptik ausgespannt, die Aspiration des jetzt viskösen Kortexgemisches wird einfacher. 3. Unabsichtliche Aspiration der hinteren Kapsel besonders zentral ist unmöglich, da die Kunstlinsenoptik vollständigen Schutz bietet. 4. Die Politur der hinteren Kapsel ist weiterhin mindestens so sicher und gut möglich wie bisher. Es werden Technik, Vor- und Nachteile sowie die klinischen Ergebnisse von 3789 Patienten demonstriert.
Summary
In more than 80% at patients, posterior capsule rupture occurs during automated cortex aspiration. If a foldable IOL is implanted at the conclusion of phacoemulsification into a mixture of viscodissected cortical remnants which have been aspirated after the implantation, inadvertent aspiration of the posterior capsule is vitually impossible, if the IOL optic is always kept between the I/A tip and the capsule. Advantages and disadvantages of this important change in phacoemulsification procedure are discussed, and the clinical results of 3789 patients are demonstrated.
