Zusammenfassung
Untersuchungsmethodik und -geräte zur Ultraschallbiometrie und Verfahren zur IOL-Berechnung wurden dargestellt. Darauf aufbauend wurden spezielle Probleme bei komplizierten Ausgangssituationen behandelt. Hierzu gehörten dichte Katarakte, lange und kurze Augen, flache und steile Hornhautradien bzw. ungewöhnliche Hornhauttopographien, z. B. nach hornhautchirurgischen Eingriffen. Weiter wurde das Vorgehen bei Silikonöltampo-nade, Pseudophakie und Tripleprozedur erörtert. Schließlich wurden allgemeine Empfehlungen zur Vermeidung von Fehlern bei Ultraschallbiometrie und IOL-Berechnung gegeben. Generell empfiehlt sich gerade auch bei Problemfällen die Verwendung der Immersionstechnik für die Achsenlängenmessung und der Einsatz theoretisch-optischer Formeln zur IOL-Bestimmung.
Summary
Examination techniques and instrumentation for ultrasonic biometry as well as algorithms for IOL calculations were described. Special problems arising from complicated initial conditions were analyzed, including dense cataracts, long and short eyes, flat and steep corneal curvatures or uncommon corneal topographies, e.g., after corneal surgery. In addition, the biometricai management of silicone oil filled eyes, pseudophakia and triple procedures was discussed. Finally, general recommendations to minimize errors in ultrasonic biometry and IOL calculations were given. The immersion A-scan technique and theoretical IOL formulae should generally be used, especially with problem eyes.
