Ergebnisse nach kombinierter Phakoemulsifikation und Goniotrepanation mittels modifizierter Skleralstanze

Zusammenfassung

Bei der Kombination einer Kataraktoperation mit einer Glaukomoperation entsprechen die unmittelbar postoperativen Beobachtungen bzgl. des intraokularen Reizzustandes (Fibrinbildung, Hyphäma, Vorderkammerabflachung) sowie der Funktion des Filterkissens häufig nicht den gewünschten Erwartungen. Um diese Ergebnisse zu verbessern, verwendeten wir eine neue Technik.

Patientengut und Methoden: An 15 Augen von 13 Patienten wurde eine Phakoemulsifikation mit Hinterkammerlinsenimplantation mit anschließender Goniotrepanation durchgeführt. Alle Augen wiesen fortgeschrittene Gesichtsfeldausfälle auf; trotz maximaler antiglaukomatöser Therapie war die Tension über 25 mm Hg. Die Goniotrepanation wurde mit einer modifizierten Skleralstanze entweder durch den Skleraltunnel hindurch (N= 9) oder nach Bildung eines Skleralläppchens (N=6) durchgeführt. Bis auf 2 Patienten mit einer Aderhautamotio zeigte sich ein reizfreier Pseudophakiestatus bei gut ausgebildeter Filtration. Im Nachbeobachtungszeitraum (maximal 7 Monate) wiesen alle Augen eine regulierte Tensiolage auf. Ein Patient benötigte eine zusätzliche lokale Therapie.

Schluβfolgerung: Mit der beschriebenen Methode und der neuen Skleralstanze kann möglicherweise die Komplikationsrate der kombinierten Katarakt-/Glaukomoperation in der frühen und späten postoperativen Phase reduziert werden.

Summary

The results of combined cataract and glaucoma operation are often not very satisfying regarding the early postoperative period: the intraocular irritation (fibrinous reaction, hyphema, decreased anterior chamber depth) and above all the function of the filtering bleb are not as expected. In order to improve this, we have developed a new technique with a scleral punch.

Patients and method: In 15 eyes of 13 patients a phacoemulsification with posterior chamber lens implantation followed by a goniotrephination was performed in the same session. All eyes had either progressed deficiencies of the visual field or a glaucomatous excavation of the optic nerve head. In spite of antiglaucomatous therapy the eye pressure was over 25 mm Hg. The goniotrephination was performed either through a scleral tunnel (N = 9) or after building a scleral flap (N = 6). Postoperatively, only two patients had a choroidal detachment, the others showed a regular pseudo-phacic eye without any irritation and with a well prepared filtration bleb. After 7 months of observation all eyes had well regulated eye pressure. One patient needed additional topical therapy.

Conclusion: We conclude that using the described method and in cooperation with our new scleral punch, the rate of complications in the early period can possibly be reduced and a successful regulation of intraocular pressure can be achieved.
