Zusammenfassung

Die klinisch-kardiologische Routine profitiert von möglichst vollständigen Einsichten in den Wirkungsmechanismus der Medikamente auf die Ventrikelfunktion. Der Nachweis von Änderungen der myokardialen Inotropie aber gerät oft nur fehlerhaft und unzuverlässig wegen des Aufwandes und der technischen Unzulänglichkeiten der damit verbundenen methodischen Bedingungen [75, 76]. An den folgenden 4 klinischen Untersuchungen kardiovaskulärer Interventionen soll die Bedeutung des Problems vorgestellt werden, nicht nur die konventionelle Hämodynamik mit ihren Änderungen, sondern auch die der Kontraktilität unter den Bedingungen der klinisch-diagnostischen Routine korrekt nachzuweisen. Anhand der Untersuchungsergebnisse soll eine praktikable Lösungsmöglichkeit für die Inotropiemessung unter klinischen Bedingungen erläutert werden.
