Schmerzen bei Krebserkrankungen — Bedeutung, Behandlung und Bewältigung

Zusammenfassung

Unter den Symptomen, die den Tumorpatienten an seine Krankheit erinnern, ist Schmerz häufig das am meisten belastende und, auch beim schmerzfreien Tumorpatienten, das am meisten gefürchtete. Bei Gesprächen mit Patienten, die über ihre Krebserkrankung informiert waren, wird immer wieder deutlich, daß nicht das Sterben, sondern das Sterben unter unerträglichen Schmerzen ihre Zukunftsängste dominiert. Von den Möglichkeiten der Schmerztherapie her beurteilt sind diese Ängste unbegründet, denn Tumorschmerzen könnten in allen Fällen so weit gedämpft werden, daß die Patienten mit den verbleibenden Schmerzen umgehen können. Trotz dieser Möglichkeit hat eine Umfrage unter Hinterbliebenen von Krebspatienten (Parkes 1978) ergeben, daß 20% der Kranken in Kliniken und 50% der zu Hause Verstorbenen unter Schmerzen gestorben sind. Auch heute, nach intensiver weltweiter Propagierung der schmerztherapeutischen Möglichkeiten, sind die Einsicht in die Notwendigkeit und die praktischen Fähigkeiten der wirksamen Behandlung von Schmerzen leider noch kein ärztliches Allgemeingut. Alle bei der praktischen Versorgung der Patienten Mitwirkenden und die Patienten selbst müssen helfen, die Situation zu ändern. Es ist deshalb für sie wichtig, die Grundzüge der Schmerzentstehung, der Schmerztherapie und der Schmerzbewältigung zu kennen.
