Summary

This paper deals with the incidence of esophageal varices in patients with myelofibrosis. Two cases are compared in detail with five cases published in the literature. The causes of esophageal varices in myelofibrosis may be splenic vein thrombosis or portal hypertension due to portal vein thrombosis, myeloid metaplasia in the liver or increased portal blood flow (so-called active portal hypertension). Esophageal varices are a rare complication of myelofibrosis. They occur in probably 1.4–5.5% of all cases. Since bleeding occurs in these patients, it would be best to treat them with aldosterone-antagonists (spironolactone). Successful surgical measures depend on the duration of illness and on the general condition of the patient.
