Laparoscopic cardiomyotomy (Heller myotomy) is performed for achalasia. The diagnostic workup must exclude several diseases that can mimic achalasia (malignant obstruction, gastroesophageal reflux with stricture formation, diffuse esophageal spasm, and nutcracker esophagus), as treatment of these is quite different. A complete diagnostic workup is outlined in Table 17.1.
