Zusammenfassung

Eines der wesentlichen Merkmale des Rechtsschenkelblocks liegt in dem initial weitgehend normalen Verlauf der Vektorschlinge nach vorne links unten (Lenègre, 1952, 1957; Wenger, 1956; Baydar, 1965). Die Orientierung der initialen Schlinge entspricht zufolge Blockierung des rechten Schenkels und Verzögerung der Erregungsausbreitung nach rechts praktisch ausschließlich den linksventriculären Partien des Kammerseptums, wobei sich die Aktivationswelle wie bei der Norm vom mittleren linksventriculären Drittel gleichzeitig nach vorne gegen die Spitze und nach hinten oben basiswärts ausbreitet (Vektor 1, Abb. 66). Durch das Überwiegen der spitzennahen Potentiale wird der manifeste septale Vektor jedoch nach vorne links orientiert. Anschließend erfolgt die Erregung der gesamten Spitzenregion sowie der Vorder- und Lateralwand des linken Ventrikels, während gleichzeitig die Depolarisationswelle langsam in das Kammerseptum eindringt und nach rechts übergreift.
