Zusammenfassung
Epibulbäre und intraokulare Tumoren unterscheiden sich in der Diagnostik und Therapie in einigen grundsätzlichen Punkten (Tabelle 1). Im Gegensatz zu Tumoren — insbesondere der hinteren Uvea — bei denen eine Probeexzision vital bedrohliche Komplikationen nach sich ziehen kann, ist eine Biopsie epibulbärer Tumoren bei leichter Zugängigkeit stets möglich. Während insbesondere maligne uveale Melanome schon früh und in über 90% in die Skiera einbrechen (Donders,1973), wachsen epibulbäre Tumoren zunächst nicht in die Skiera — ein entscheidender Gesichtspunkt für die Planung einer mehr konservativen, bulbuserhaltenden Therapie. Dagegen ist bei malignen epibulbären Tumoren stets eine Absiedlung in die regionalen präaurikulären oder submandibulären Lymphknoten zu befürchten und bei der Therapie mit in Betracht zu ziehen. Eine regionale Lymphknoten-Beteiligung bei intraokularen Tumoren kommt praktisch kaum vor.
