Zusammenfassung
Alle Formen von Schädigung oder Gefährdung eines Patienten, die auf den Einfluß eines Krankenhausaufenthaltes zurückzuführen sind, werden als Hospitalismus bezeichnet (Kunz, 1957). Die Begriffsskala reicht demnach von soziologisch-psychologisch schädlichen Einflüssen bis zur somatischen Schädigung oder Gefährdung des Patienten selbst oder auch seiner Umgebung. Die Hospitalinfektionen, die heute unter dem Begriff Hospitalismus im engeren Sinne verstanden werden, stellen weitaus die größte Gefährdung des Patienten dar (Spitzy, 1963). Man kann aber bei Hospitalinfektionen erst dann von Hospitalismus sprechen, wenn das zahlenmäßige Auftreten von gleichartigen Infektionen im Krankenhaus wesentlich über das in der übrigen Bevölkerung hinausgeht (Meyer, 1962). Gelegentliche, auch gehäufte Infektionsfälle in Krankenhäusern sind noch keineswegs ein Grund, Hospitalismus anzunehmen. Schließlich handelt es sich um Kranke, die anfällig für Infektionen sind. So kann eine Virusinfektion beispielsweise die Patienten in einem Zimmer oder einer Abteilung erfassen. Jeder in einem Krankenhaus tätige Arzt kennt das gelegentliche Auftreten von Fieberschüben bei mehreren Patienten. Es soll die Gefahr der Spitalsinfektion nicht übertrieben werden. Hospitalismus im engeren Sinne liegt erst dann vor, wenn eine exakte Analyse ergeben hat, daß sich Erreger im Krankenhausmilieu durchgesetzt haben, auf die Kranken übertragen werden und sich dort ansiedeln. Hier lösen sie eine Infektion aus oder machen die Kranken zu Keimträgern und damit zu einer Gefahr für ihre Umgebung.
