Abstract

Spontaneous rupture of the esophagus can be defined as a complete tear of all layers in an apparently normal gullet. A distinction must be made between perforation and rupture. A perforation results from a lesion, iatrogenic or not, or is a final stage of an esophageal disease. What is described as a spontaneous rupture is mostly caused by vomiting and is therefore called „primary pressure rupture of the esophagus“ (Moynihan, 1954). The term „secondary pressure rupture of the esophagus“ (Toghill et al., 1968) is used when other causes of increased intraabdominal pressure, such as a status asthmaticus (Raffle, 1958) or epilepsy (Klein and Grossman, 1943) produce the rupture. In some cases the rupture seems to be „really spontaneous“; Heroy (1952) described a case of spontaneous rupture of the esophagus which occurred during T.V. watching and Clark and Tankel (1964) described one that took place while the person was asleep.
