Abstract
The association between carotid artery disease and stroke has long been recognized. Artery to artery embolism is one of several mechanisms causing either transient ischemic attacks (TIAs) or stroke ipsilateral to atherosclerotic plaques [25]. Thrombus at the site of carotid arterial occlusion [12], atherosclerotic debris, and vertebral artery stenosis or occlusion are well-documented sources of emboli [26]. Fibrin-platelet thrombus forms in carotid artery plaques with severe stenosis. These thrombi, when partially or totally dislodged, can produce distal embolism. Ulceration, a frequent finding in carotid plaques, exposes atherosclerotic plaque matrix to flowing blood and leaves behind cavities (changes in plaque morphology) within the plaques, inducing local flow disturbances; both factors independently or associated can trigger thrombosis.
