Behandlung der Hämochromatose

Zusammenfassung

Die Therapie der Hämochromatose geht von 2 Annahmen aus: 1. Die erhöhte Eisenspeicherung wirkt gewebstoxisch und ist für die typischen klinischen Symptome chronische Lebererkrankung, Diabetes mellitus, Sterilität und Hauptpigmentierung verantwortlich. Für diese Annahme gibt es jedoch im Tierexperiment keine Beweise, denn trotz massiver Eisenzufuhr und Ablagerung im Gewebe läßt sich kein der Hämochromatose entsprechendes Krankheitsbild erzeugen. Erst in letzter Zeit konnte bei Kindern, die wegen einer Thalassämie regelmäßig Bluttransfusionen erhielten, gezeigt werden, daß bei Körpereisendepots (normal < 5 g) von (10)-20 g ab klinische Symptome auftreten und bei 60 g transfundiertem Eisen (nach 140–150 1 Blut) der Tod eintritt. Histologisch und elektronenmikroskopisch lassen sich auch im Tierexperiment Störungen an den Zellorganellen nachweisen, die zu biochemischen Störungen und zur Freisetzung lysomaler Enzyme führen und Grundlage der wichtigsten klinischen, morphologischen und biochemischen Symptome der Hämochromatose sein können [2, 7, 11, 16].
