Summary
In a randomized, cross-over, controlled clinical study, a total of 23 patients with chronic atrial fibrillation was treated with disopyramide 100 mg t. i. d. for 3 days and 200 mg t. i. d. for the following 7 days or with cordichin (160 mg quinidine and 80 mg verapamil), 1 tablet t.i.d. for 3 days and 2 tablets t. i. d. for 7 days. Out of 20 cases (15 men, five women) a medication-induced conversion to sinus rhythm was achieved in 17 patients with cordichin. Echocardiographic measurements in these patients showed a decrease of the systolic ventricular diameter, no change in diastolic diameter, an increase in fractional shortening of the left ventricle, in the systolic thickness of the posterior left ventricular wall, in the mean and maximal velocity of the posterior wall motion in systole and diastole as well as normalized motion of the posterior wall. The duration from the beginning of the ventricular phase to maximal motion did not change. There was no change in the normalized PEP and LVET values. VCF mean and VCF max and delta VCF increased. Under treatment with disopyramide, no changes of the initial values were recognized.
The results show that after digoxin, the often described negative inotropic effects of the three substances did not appear and that medication-induced conversion to sinus rhythm using cordichin led to a marked improvement of left ventricular pump function.
Zusammenfassung
An 23 Patienten mit chronischem Vorhofflimmern wurde nach einem Cross-over-Randomisierungsschema eine medikamentöse Therapie mit Disopyramid 3 x 100 bis 3 x 200 mg/die bzw. Cordichin (160 mg Chinidin + 80 mg Verapamil) 3 x 1 bis 3 x 2 Tbl./die über insgesamt jeweils 10 Tage durchgeführt. In 20 Fällen (15 männliche, 5 weibliche Patienten) wurde unter Cordichin eine medikamentöse Rhythmisierung erzielt. Bei diesen Patienten wurde mit Hilfe der echokardiographi- schen Messungen eine Abnahme des systolischen Ventrikeldurchmessers bei gleichbleibendem diastolischem Durchmesser, eine Zunahme der Verkürzungsfraktion des linken Ventrikels, eine Zunahme der systolischen Hinterwanddicke und eine Steigerung der mittleren und maximalen systolischen Hinterwandsteigung sowie diastolischen Hinterwanderschlaffung und auch der normalisierten Hinterwandbewegung festgestellt. Die Zeiten vom Beginn der Systole bis zum Erreichen der maximalen Steigung sowie vom Beginn der Diastole bis zum Erreichen der maximalen Erschlaffung änderten sich nicht. Ebenso konnte keine Veränderung der systolischen Zeitintervalle, auch nicht der frequenzkorrigierten Werte, gefunden werden. Die VCFmean- VCFmax und Delta-VCF nahmen signifikant zu. Für sämtliche Parameter fand sich unter Disopyramid keine Änderung im Vergleich zum Ausgangswert der Patienten. Die Ergebnisse zeigen, daß bei vorangehender Digitalisierung die vielfach beschriebenen negativ-inotropen Eigenschaften der angewendeten Substanzen nicht manifest wurden und im Falle der medikamentösen Rhythmisierung durch Cordichin eine deutliche Verbesserung der Pumpfunktion auch unter diesem Pharmakon resultierte.
