Antifibrillatorische Wirkung von Chinidin/Verapamil und Chinidin/Pindolol bei Vorhofflimmern

Zusammenfassung

Die Feststellung der antifibrillatorischen Wirksamkeit von Chinidin/Verapamil bei chronischem Vorhofflimmern erfolgte in einer retrospektiven Studie mit Auswertung der Behandlungsergebnisse von insgesamt 200 Patienten, die zur Rhythmisierung stationär aufgenommen wurden. Bei 25 Patienten mit Chinidin-refraktärem Vorhofflimmern konnte durch zusätzliche Gabe von Verapamil ein Sinusrhythmus erreicht werden. Von den insgesamt 142 Patienten, die sofort mit der Kombinationstherapie Chinidin/Verapamil behandelt wurden, konnten 110 Patienten rhythmisiert werden. Im Gegensatz zur Therapie mit Chinidin/Verapamil war die Kombinationsbehandlung mit Chinidin/Pindolol enttäuschend. Nur einer von 17 Patienten konnte rhythmisiert werden, obwohl alle Patienten ausreichend lange mit der Kombination behandelt wurden. Die Untersuchungen zeigen in Übereinstimmung mit früheren Befunden aus dieser Klinik und Beobachtungen anderer Autoren, daß durch kombinierte Gabe von Chinidin und Verapamil bei Vorhofflimmern ein über die Wirkung der Einzelsubstanzen hinausgehender erheblich überadditiver Effekt erreicht wird. Bei Kombination von Chinidin mit Pindolol wird kein der Chinidin/Verapamilwirkung entsprechender antifibrillatorischer Effekt erreicht.

Summary

The antifibrillatory efficacy of quinidine/verapamil in chronic atrial fibrillation was assessed in a retrospective study. The results of treatment of a total of 200 patients who had been admitted to hospital for restoration of sinus rhythm were evaluated. The atrial fibrillation refractory to quinidine of 25 patients reverted to sinus rhythm after additional administration of verapamil. Sinus rhythm was restored in 110 of the total of 142 patients who were treated from the outset with the combination quinidine/verapamil. Contrary to the treatment with quinidine/verapamil, combination treatment with quinidine/pindolol was disappointing. Sinus rhythm was restored in only one of 17 patients although all patients were treated with the combination for a sufficient period of time. In agreement with earlier results from our clinic and observations of other authors the investigations show that a considerable over-additive effect as compared to the action of the substances alone is obtained when quinidine and verapamil are given in combination for the treatment of atrial fibrillation. The antifibrillatory effect achieved with the combination quinidine/pindolol is by no means comparable with that of quinidine/verapamil.
