Zusammenfassung
Die außerordentliche Häufigkeit von Kammerflimmern als Ursache plötzlicher Todesfälle bei ischämischer Herzkrankheit begründet die dringende medizinische Notwendigkeit einer wirksamen Prophylaxe bei entsprechend gefährdeten Patienten. Die therapeutischen Bemühungen sind jedoch besonders dadurch erschwert, daß weitgehende Unsicherheit in der Frage besteht, welche antiarrhythmischen Maßnahmen im Einzelfall geeignet sind, die Entstehung von Kammerflimmern zu verhindern. Faßt man die bisherigen experimentellen Beobachtungen und klinischen Erfahrungen zur Frage des Zusammenhanges zwischen antiarrhythmischer Wirksamkeit und antifibrillatorischem Effekt zusammen, so lassen sich die folgenden Hypothesen aufstellen:

Kammerflimmern entsteht aus einer Vielzahl verschiedener Mechanismen, die klinisch nur teilweise zu trennen sind. Eine Prävention der Arrhythmie erfordert daher differentialtherapeutische Überlegungen und Maßnahmen.

Zwischen antiarrhythmischer und antifibrillatorischer Wirkung besteht dann ein eindeutiger Zusammenhang, wenn die antiarrhythmische Wirkung einzelne oder mehrere Auslösemechanismen des Kammerflimmerns erfaßt.

Antiarrhythmika im engeren Sinne, d. h. Lokalanästhetika, beeinflussen nur einen Teil der bei ischämischer Herzkrankheit bedeutsamen Auslösefaktoren. Dies trifft für chronisch rezidivierende ventrikuläre Tachykardien zu, möglicherweise auch für chronische, gehäuft auftretende repetitive bzw. hochvorzeitige Ektopien.

Eine generelle Prophylaxe von Kammerflimmern bei ischämischer Herzkrankheit mit Lokalanästhetika ist nicht möglich. Bei instabilen Arrhythmien, die als Folge abrupter Änderungen der ventrikulären Vulnerabilität entstehen, haben diese Substanzen nur begrenzten Wert. In dieser Situation ist allerdings die antiarrhythmische Wirksamkeit keine notwendige Voraussetzung für den antifibrillatorischen Effekt.
Summary
As sudden death in ischaemic heart disease is very often attributable to ventricular fibrillation, effective prophylactic tíeatment of the jeopardized patients is of the utmost medical importance. However, therapeutic efforts are complicated in particular by the fact that more often than not it is quite unclear which antiarrhythmic measures should be taken in each particular case to prevent ventricular fibrillation. Based on the experimental observations and clinical experience on the correlation between antiarrhythmic efficacy and antifibrillatory effect gained so far the following hypotheses can be postulated:

Ventricular fibrillation is caused by a variety of mechanisms which clinically can only be separated to a certain degree. Hence, prevention of arrhythmia requires differential diagnostic considerations and measures.

There is a clear correlation between antiarrhythmic and antifibrillatory effects if the antiarrhythmic effect counteracts one or several mechanisms which elicit ventricular fibrillation.

3.
Antiarrhythmics in the strict sense, i.e. local anaesthetics, affect only some of the important factors which elicit ischaemic heart disease. This is true for chronic recurrent ventricular tachycardia, possibly also for chronic frequent recurrent or premature ectopic beats.

General prophylactic treatment for the prevention of ventricular fibrillation in ischaemic heart disease is not possible with local anaesthetics. In unstable arrhythmias which are the result of abrupt changes of ventricular vulnerability these substances are of limited value only. In this condition, however, the antiarrhythmic efficacy is not necessarily a pre-condition for the antifibrillatory effect.
