Avulsion Fractures of the Intercondylar Eminence in Adolescents: Pathophysiology, Case Material, and Late Results

Abstract

Ligamentous injuries in children are rare. A relatively common type is the avulsion fracture of the intercondylar eminence, whose causative mechanism is identical to that of cruciate ligament tears. It is the typical injury of the central pivot in the pediatric age group (Bachelin and Pirkls 1987; Baxter and Wiley 1988; Dejour 1982; Rigault et al. 1976; Seriat-Gautier 1983). The fracture is caused by the relatively high mechanical resistance of the cruciate ligaments compared with their bony sites of insertion. Nevertheless, isolated midsubstance ligament tears are occasionally encountered in adolescents whose growth plates have not yet closed (Bradley et al. 1979; Dejour 1982). Both types of injury - midsubstance tears and bony avulsions - are caused by similar mechanisms and present identical subjective symptoms and clinical signs on initial consultation.
