Diagnosis of Acute Tears of the Anterior Cruciate Ligament, and the Clinical Features of Chronic Anterior Instability

Abstract

In an acutely injured or chronically unstable knee, the correct diagnosis of anterior cruciate ligament (ACL) insufficiency is based on the mechanism of the injury as recalled by the patient, clinical stability testing, and diagnostic arthroscopy. The most commonly injured ligament of the knee joint is the ACL. Diagnosis is not a simple matter. Noyes et al. (1978) report that in 103 patients with chronic symptomatic anterior instability, the first examining physician correctly diagnosed a torn anterior cruciate ligament in only 7 cases — truly a disturbing statistic. The slightest suspicion of a tom anterior cruciate ligament should prompt us to exhaust all available means to establish a diagnosis within 1 week.
