Percutaneous Diskectomy and Chemonucleolysis

Abstract

The degree of reliability and standardization of percutaneous diskectomy and chemonucleolysis achieved over the last few years has led these techniques to occupy a distinct and important position in the range of therapeutic options that can be employed to treat lumbar disk herniation. The role played by the neuroradiologist has furthermore gradually superseded that of the orthopedist or neurosurgeon in carrying out the basic manual procedure, namely puncture of the intervertebral disk under fluoroscopic monitoring. This procedure, though relatively simple, requires not only precise anatomic knowledge but also the so-called radiologic mentality that is indispensable for optimum exploitation of the equipment and superior orientation when using fluoroscopy. In effect, as we shall see below, close collaboration between clinicians and radiologists is indispensable for good results, but the percutaneous treatment of the herniated disk may rightfully be considered the domain of therapeutic neuroradiology (or interventional neuroradiology, as it may be called).
