Abstract
Most patients with hypertrophic cardiomyopathy come to medical attention because of symptoms of effort dyspnea, chest pain, fatigue, and alterations in consciousness. Although features of this disease such as abnormal diastolic filling properties and outflow obstruction may account for many of these symptoms, the presence of chest pain may indicate myocardial ischemia. However, unlike patients with coronary artery disease, most symptomatic patients with hypertrophic cardiomyopathy have normal (and often enlarged) epicardial coronary arteries, indicating a different mechanism for ischemia in hypertrophic cardiomyopathy, if present.
