Zusammenfassung

Als Retinoide bezeichnet man die natürlich vorkommenden Vitamin-A-Substanzen (Vitamin-A-Alkohol = Retinol, Vitamin-A-Aldehyd = Retinal und Vitamin-A-Säu- re = Retinsäure) sowie deren synthetische Abkömmlinge [36]. Die Herstellung der letzteren entsprang ursprünglich dem Wunsch nach Vitamin-A-Präparaten mit günstigerem therapeutischen Index. Bislang wurden schon mehr als 1500 solche Retinoide synthetisiert und in verschiedenen in vivo- und in vitro-Testsystemen erprobt [1]; viele davon besitzen eine Reihe potenter und zum Teil völlig neuartiger klinischer Wirkungen, die die gesamte Stoffklasse in das Zentrum der Aufmerksamkeit gerückt haben. Zwei synthetische Retinoide sind in vielen Ländern seit kurzem im Handel: Isotretinoin (13-Cis-Retinsäure) (Accutan), ein in geringen Mengen vorkommender natürlicher Vitamin-A-Metabolit, der jedoch schon vor seiner Entdeckung im Organismus synthetisiert und im Tierversuch erprobt wurde; und Etretinat, ein aromatisches Retinoid (Tigason). Diese beiden Substanzen sind einander in ihrer Wirkungsweise zwar ähnlich, doch bestehen erhebliche Unterschiede in Anwendungsbereich und Pharmakodynamik (Tabelle 1).
