Complications of Total Abdominal and Spleen Irradiation in Patients With Lymphomas

Abstract

Exploratory laparotomy has shown that, in patients with non-Hodgkin’s lymphomas, involvement of para-aortic nodes is associated with positive mesenteric nodes in 80% of well- differentiated nodular or diffuse lymphocytic lymphomas and in lymphocytic lymphomas and in 50% of cases of histiocytic lymphoma [10]. Current treatment regimens stipulate that when there is supra- or infradiaphragmatic involvement, “icebergs” shall be irradiated after initial chemotherapy. Thus, in addition to the classic mantle and inverted Y treatments, we now routinely have to irradiate the whole abdomen, and this may present problems because of the large volume to be irradiated, the sensitivity of healthy organs within the field, and previous treatments which the patients have received.
