Zusammenfassung
Die glaukomatöse Gesichtsfeldschädigung ist hauptsächlich vom intraokularen Druck, aber auch von lokalen und systemischen Faktoren wie Blutdruck, Vasosklerose, Blutviskosität abhängig [3,10,14,15,16]. Es wird davon ausgegangen, daß beim Glaucoma simplex ein vorwiegend augeninnendruckab-hängiger Mechanismus der Gesichtsfeldschädigung vorliegt, während beim Glaukom ohne Hochdruck ein vorwiegend vaskulärer Faktor die Gesichtsfeldschädigung bewirkt Bei den langsam progredient fortschreitenden ischämischen Neuropathien, die keine Papillenexkavation zeigen, steht eine druckunabhängige, vaskuläre Ursache im Vordergrund. Alle drei Erkrankungen zeigen in den Anfangsstadien ähnliche Formen der Gesichtsfeldausfälle, die durch den Nervenfaserverlauf geprägt sind, wie Abb. 1 zeigt. So finden sich anfangs oft kleine, isolierte parazentrale Skotome, die noch keine Verbindung zum blinden Fleck aufweisen.
