Corneal Perforation in Herpes Zoster Ophthalmicus Caused by Eyelid Scarring With Exposure Keratitis

Summary

We present four cases of corneal perforation in herpes zoster ophthalmicus. In all cases, corneal exposure, hypesthesia, and treatment with topical corticosteroids caused the perforation. Corneal perforation can occur without zoster keratouveitis. Vigorous early prophylaxis such as tarsorrhaphy, lubrication, and soft contact lenses can prevent these disasters.

Zusammenfassung

Wir berichten über vier Patienten mit einer Hornhaut-perforation nach Zoster ophthalmicus. In allen Fällen verursachten ein ungenügender Lidschluß mit Hornhaut-exposition, Hypaesthesie und lokale Steroidbehandlung diesen schweren Verlauf. Eine Hornhaut-perforation kann auch ohne Zoster-Keratouveitis eintreten. Nur mit energischen und frühzeitig einsetzenden Maßnahmen wie Tarsorrhaphie, Salbenabdeckung und weichen therapeutischen Kontaktlinsen kann man solch eine schwere Komplikation abwenden.
