Laparoskopische Versorgung biliodigestiver Fisteln

Summary

In about 800 endoscopic operations we found 12 patients with 14 biliodigestive fistulas, including cholecystocolonic, jejunal, duodenal and gastric fistulas. The treatment of eight patients has performed by laparoscopic technique. After surgical exposure the following interventions are recommended: Lock with the Röder-loop or ligature with extracorporal knot, cover with Endo-GIA and manual seam. Fibrin-sticking is a complementary application. The simple or double Röder-loop is the simplest variant, but not possible in all cases. If the closure is insecure or if there is partial damage to the wall, the treatment can be complemented by fibrin-sticking or a manual seam. If the base of the fistula is wide, we recommend ligation of the fistula at its base of the application of an Endo-GIA after sufficient preparation. The preparation and representation of the dorsal wall is important.

Zusammenfassung

Bei etwa 800 endoskopischen Operationen fanden wir 14 biliodigestive Fisteln bei 12 Patienten: -cholezystokolische, -jejunale, -duodenale und -gastrische Fisteln, deren Versorgung bei 8 Patienten auf laparoskopischem Wege möglich war. Folgende Operationsverfahren sind zu empfehlen: Der Fistelverschluß mit der Röder-Schlinge oder durch extrakorporale Knotung, die Versorgung mit dem Endo-GIA und die manuelle Naht. Die Fibrinklebung kann ergänzend angewandt werden. Die einfache oder doppelte Röderschlinge ist am einfachsten, doch nicht immer möglich. Bei unsicherem Verschluß oder partieller Wandschädigung kann die Versorgung durch Fibrinklebung oder manuelle Naht ergänzt werden. Bei breitbasigen Fisteln empfiehlt sich der basisnahe Verschluß oder nach übersichtlicher Präparation der dorsalen Wand die Anwendung des Endo-GIA.
