Zusammenfassung

Während vor Einführung der transkraniellen Dopplersonographie intrakranielle Gefäßstenosen lediglich indirekt — beispielsweise über einen erhöhten Gosling-Widerstandsindex bei Beschallung der extrakraniellen Gefäße — vermutet werden konnten, ermöglichte die TCD den Nachweis umschriebener Beschleunigungen der Blutflußgeschwindigkeit (CBFV) in den großen basalen Hirnarterien mit Veränderungen des Frequenzspektrums, so daß es möglich war, die Verdachtsdiagnose intrakranieller Stenosen mittels TCD direkt zu stellen. Zu den diagnostischen Kriterien für eine Lumeneinengung von mehr als 50% zählen in Analogie zur extrakraniellen Dopplersonographie:

  • die umschriebene Zunahme der mittleren und systolischen Strömungsgeschwindigkeit

  • Verschiebung des Frequenzspektrums in den niederfrequenten Bereich mit Refluxphänomenen und

  • Lokalisation der Strömungsbeschleunigung außerhalb der Verbindungsarterien (A. communicans anterior, A. communicans posterior) bei Kranken mit hämodynamisch relevanten extrakraniellen Strömungsbehinderungen.
