Zusammenfassung
Nach Herz- und Lungentransplantationen stellt eine pulmonale Hypertonie (PH) eine schwer therapierbare Komplikation dar. Hypoxie, Hyperkapnie sowie die unphysiologische Situation des kardiopulmonalen Bypasses können zur Erhöhung des pulmonalvaskulären Widerstandes (PVR) beitragen [1] und damit zu einer Überlastung des rechten Ventrikels führen. Bei Lungentransplantationen können Ischämie- und Reperfusionsschäden des Transplantats eine pulmonale Hypertonie zusätzlich fördern. Weiterhin kann ein suboptimales Ventilations-Perfusions-Verhältnis zu einer reduzierten Oxygenierung fuhren, so daß die Myokardkontraktilität durch ein vermindertes Sauerstoffangebot beeinträchtigt wird.
