Zusammenfassung
Auch der Rückfuß wird bei Patienten mit rheumatoider Arthritis allmählich in den Krankheitsprozeß einbezogen. Der Befall kann nur geringgradig ausgeprägt sein, jedoch auch rasch fortschreiten. Häufig ist der Vorfuß mitbetroffen. Im Verlauf der Sehnenscheiden über dem Sprunggelenk sowie über der Sehne des M. tibialis posterior und gelegentlich auch entlang der Peronäalsehnen findet sich eine Schwellung. Die Schwellung der Rückfußgelenke kann am besten auf der Medialseite des Talonavikulargelenks und über dem Sinus tarsi auf der Lateralseite beobachtet werden. Die Synovialitis der Rückfußgelenke, der damit verbundene Verlust des hyalinen Gelenkknorpels, Erosionen des talonavikularen und des subtalaren Gelenks und wahrscheinlich auch der Funktionsausfall der Sehne des M. tibialis posterior führen zu einer verbleibenden Valgusdeformität des Rückfußes, die man bei 80% der Patienten mit Beteiligung des Rückfußes findet. Das Talonavikulargelenk wird instabil, und das Caput tali verlagert sich nach medial und plantar [7, 12]. Der Rest von Mittel- und Vorfuß verschiebt sich in Abduktionsstellung. Der Kalkaneus kippt in Valgusstellung und kann so an die Fibulaspitze anstoßen, was zu Schmerzen im Bereich des Außenknöchels [9] und manchmal zu einer Streßfraktur der distalen Fibula führt (Abb. 1). Die Sehne des M. tibialis posterior kann reißen [11] oder sie kann in ihrer Funktion eingeschränkt sein und damit als medialer Stabilisator des Rückfußes ausfallen. Nur 8% aller Patienten mit einer Krankheitsdauer von weniger als 5 Jahren hatten mäßige oder schwere Deformitäten des Rückfußes.
