Zusammenfassung

Das Kehlkopfkarzinom ist mit einem Anteil von weniger als 2% an allen Malignomen ein insgesamt seltener Tumor. Nach Schätzung des Bundesgesundheitsamtes erkrankten 1991 etwa 2800 Personen in den alten Bundesländern an einem Larynxkarzinom. Es zählt jedoch zu den häufigsten Tumorlokalisationen im oberen Aerodigestivtrakt. Der Altersgipfel liegt im 5. bis 7. Lebensjahrzehnt, Männer sind wesentlich häufiger betroffen (Geschlechtsverhältnis 4,5 bis 6 zu 1). Dies ist auf die erhebliche Bedeutung von Umweltnoxen, in erster Linie den Tabakabusus, bei der Entstehung des Larynxkarzinoms zurückzuführen. Eine zusätzlich potenzierende Wirkung durch gleichzeitigen Alkoholkonsum ist gesichert, scheint jedoch weniger ausgeprägt als bei anderen Plattenepithelkarzinomen des Kopf-Hals-Bereichs. Das Verhältnis von supraglottischen zu rein glottischen Karzinomen zeigt erhebliche geographische Unterschiede. Dies ist wahrscheinlich auf die unterschiedliche Verbreitung der angesprochenen Noxen zurückzuführen (Million et al. 1994).
