Offene Unterschenkelfrakturen: Bringt die interfragmentäre Zugschraube bei der Versorgung einen Behandlungsvorteil?

Summary

In a retrospective study, fixation and healing time, number of consecutive procedures, rate of complications, and the overall results of 31 open tibial shaft fractures (OTS#) in 28 patients that were stabilized and definitely treated using an external fixator (Ex. Fix.) and interfragmentary lag screw fixation (ILS) were compared with identically rated 54 OTS# in 51 patients that were stabilized by an Ex. Fix. only. Patients that treated with the combination osteosynthesis showed a prolonged fixation/ healing time, an increase in the rate of secondary procedures and complications, and an inferior overall results. Except for a temporary application, there seem to be no advantages in additionally using ILS to treat OTS# stabilized by an Ex. Fix.

Zusammenfassung

Die Fixations- und Heilungsdauer, Zahl der weiteren Eingriffe, Komplikationsrate und die Behandlungsergebnisse von 31 offenen Unterschenkelschaftfrakturen (USS#) (bei 28 Patienten), die mit einem Fixateur Externe (Fix. Ex.) und interfragmentären Zugschrauben (IFZ) stabilisiert und ausbehandelt worden waren, wurden mit als identisch klassifizierten 54 offenen USS# (bei 51 Patienten), die nur mit einem Fix. Ex. stabilisiert und ausbehandelt worden waren, verglichen. Die Kombinationsosteosynthese zeigte eine verlängerte Fixations- und Heilungsdauer, erhöhte Zahl an notwendigen weiteren Eingriffen, vergrößerte Komplikationsrate und ein insgesamt schlechteres Ergebnis. Die zusätzliche Osteosynthese mit IFZ ist bei offenen USS# nicht mehr zu empfehlen.
