Zusammenfassung
Burrows u. Tartter [6] berichteten 1982 erstmals über eine Verschlechterung der Prognose bei Patienten mit kolorektalen Karzinomen, die eine perioperative Bluttransfusion erhalten hatten. Sie gaben für die transfundierten Patienten eine Fünfjahresüberlebensrate von 51% vs. 84% bei den Patienten ohne Bluttransfusion an (p<0,005). Während diese Beobachtungen in den folgenden Jahren durch einige ähnliche Arbeiten gestützt wurden [4, 8, 10, 25, 27, 31, 35], kamen andere Arbeitsgruppen zu dem Ergebnis, daß der Einfluß der Bluttransfusion auf die Prognose maligner Tumoren kein eigenständiger ist, sondern lediglich Ausdruck damit assoziierter negativer Prognosefaktoren [2, 8, 9, 16, 21, 22, 30, 35, 36]. Im folgenden sollen diese Zusammenhänge näher analysiert, und generell Ansätze zur Vermeidung der Fremdbluttransfusion auch bei Patienten mit Tumorleiden erörtert werden.
