Photoallergie und chronische aktinische Dermatitis — ein neues Konzept

Zusammenfassung

Photoallergie und chronische aktinische Dermatitis sind Anfangs- und Endzustände in einem Spektrum von Erkrankungen, die mit einer akuten Photosensibilisierung oder einer chronisch persistierenden Photosensitivität einhergehen. Klinisch stehen Lichtempfindlichkeit und ekzematöse Hautverände rungen in lichtexponierter Haut im Vordergrund. In der Literatur sind seit der Erstbeschreibung der per sistierenden Lichtreaktionen 1962 durch Wilkinson [25] verschiedene Dermatosen mit chronischer Pho tosensitivität beschrieben und definiert worden. Hierzu gehören neben der persistierenden Lichtre aktion das aktinische Retikuloid, das photosensitive Ekzem, die chronische photosensitive Dermatitis und die lichtaggravierte atopische Dermatitis. Eine eindeutige Zuordnung anhand der historisch gege benen Definitionen ist jedoch im Einzelfall nicht im mer möglich. 1979 haben daher Hawk und Magnus [7] als zusammenfassendes Konzept die chronische aktinische Dermatitis vorgestellt. Ein auf diesen Vorschlägen aufbauendes erweitertes Konzept und seine Beziehung zur Photoallergie sollen nachfol gend dargestellt werden.
