Zusammenfassung
Thrombozytäre Antigene, die ein Fet von seinem Vater geerbt hat, können zur Immunisierung der Schwangeren führen, wenn das Antigen auf ihren Thrombozyten nicht vorkommt. Plazentagängige mütterliche IgG-Antikörper gelangen in den fetalen Körper und zerstören dort die Thrombozyten [1]. Die Erkrankung kann aufgrund des frühen Übertritts der fetalen Antigene bereits in der ersten Schwangerschaft auftreten. Die verbesserten diagnostischen Möglichkeiten haben dazu geführt, daß die neonatale Alloimmunthrombozytopenie mittlerweile bei einem von 2000 Lebendgeborenen festgestellt wird [2] — wesentlich häufiger als früher vermutet. PlA1 mit 85% und Bra mit 12% sind die thrombozytären Antikörper, die am häufigsten bei NAIT gefunden werden (Tabelle 1 [3]). In etwa 10–20% der Fälle treten Hirnblutungen mit letalem Ausgang oder bleibenden neurologischen Schäden auf [4].
